Please cancel at least 24 hours in advance to avoid a late cancellation fee.

Cancel or reschedule an appointment

Including sick notes, insurance forms, and other documentation. We’ll let you know if an appointment is required.

Request a form, note, or paperwork

Running low on medication? Book an appointment for the prescription and associated medical monitoring. For short-term convenience, your pharmacy can fax us a renewal request. A $30 fee applies, and may take up to 5 business days after payment.

Renew my prescription

If more than two weeks have passed and you have not heard from us, submit a follow-up inquiry.

Request test results

After referrals are sent, specialists usually contact patients directly. If you haven’t heard within 4 weeks (non-urgent), or 2 weeks (urgent), submit a follow-up inquiry.

If you have any questions or information to share with your doctor, please book an appointment. Direct messaging with the doctors is unavailable.
